The following questionnaire will provide some basic information to start preparing and planning your ceremony.
1. Do you have a location picked out?
If so is the ceremony going to be indoors or outdoors?
If not do you need assistance in locating a ceremony location?
2. How long would you like the ceremony to last?
3. How many guests do you expect to attend?
4. What type of vows would you like to have?
Do you need help with the vows?
5. Do you plan on having a rehearsal session?
6. Do you plan on using microphones or a PA system during the ceremony?
7. Do you have a preference on the attire of the officiant? If so please specify.
8. Do you need any witnesses provided during the ceremony?
9. Do you plan on adding anything during the ceremony such as a unity candle lighting?
10. Do you know all marriage requirements in your State and County? Most States have a waiting period from the date you apply for a marriage license.
